Your Elder Medical Care Team
The Elder Medical Care team is comprised of over 40 nurse practitioners, physicians and social workers who are able to serve as primary care providers for you or your loved one. Our geriatricians provide medical oversight and coordinate treatment plans in multiple settings.
Your Team Consists of:

Physicians / Nurse Practitioners
Physicians and nurse practitioners provide primary care. For instance, they will assess your health and manage your symptoms. In addition, they prescribe and oversee medications. Further, they provide supportive care. For example, they assist with advance care planning and guidance. They may coordinate with other in-home or residential care services.

Case Manager
To begin with, the nursing case manager or social worker will fully assess your needs. In addition, they help you decide on your goals of care. With this in mind, they will create and coordinate a personal plan of services. They will visit in person and follow up with you by phone.

Volunteers
Volunteers can be a helping hand or a companion. They provide respite breaks for caregivers. In addition, they may help with errands or light housework. For example, they may make a meal. Most important, all volunteers are fully screened and go through thorough training.

Chaplains for Palliative Care
Chaplains can visit at select locations for our Palliative Care patients. They provide spiritual support for patients and families facing serious illness.

Other Team Members
For those receiving Rehabilitation services, other team members may include therapists (speech, physical, occupational and activity), dietitians and pharmacists.
Caregiver Help in the Home
